Health Consultations

Health consultations and health education
Sukoyaka consultations are available at both the Tamaho and Tatomi health administration centers and involve one-on-one conversations with public health nurses on health-related matters. The city also provides health education outreach programs in the local community in order to spread knowledge and related technologies for preventing lifestyle diseases and enhancing city residents' health.
In addition, the city also issues health handbooks on demand to record the results of health examinations. Please stop by to pick up your handbook to better monitor your health.
*The Health Promotion Division offers consultations regularly during normal business hours over the phone or in person, including consultations regarding mother and child related matters.

Medical Care Cost Reimbursements

Lump-sum allowance for childbirth and nursing
This reimbursement is provided to eligible National Health Insurance enrollees after childbirth.
Eligible Persons Reimbursement Amount Application Procedure (Direct-Pay System)
Must be enrolled in National Health Insurance or dependent of someone enrolled and give birth at least 4 months (85 days) after pregnancy (including stillbirths and miscarriages, etc.) 420,000 yen (as of April 1, 2011)
(390,000 yen for births other than those that take place in medical institutions enrolled in the obstetrics medical care guarantee program after a 22-week gestation period)
Show your National Health Insurance card to the medical institution where you receive treatment and sign the agreement form for the direct-pay system.
*When the payment amount is less than the cost of the birth, National Health Insurance enrollees can claim the difference after filing an application with their medical insurer.
*When not using the direct-pay system, National Health Insurance enrollees will be paid the reimbursement amount after birth and after filing an application with their medical insurer.

Medical care cost reimbursement program for city residents with severe physical disabilities
This program covers the National Health Insurance self-pay cost for city residents with severe physical disabilities whenever they receive treatment at a hospital. Those eligible and wishing to enroll in the program must have their legal guardian complete an application form to receive a medical care certificate.

Children's medical care subsidy
Children up to the 6th grade of elementary school are eligible to receive free medical care at medical institutions located in Yamanashi Prefecture. Please note that only medical care costs covered under National Health Insurance are eligible.
Eligible Persons Scope of Subsidy Application Period Required Documents
Children living in the city between the ages of newborn up to the 6th grade of elementary school (the end of the 6th grade school year) Medical care costs covered under National Health Insurance (Please show your children's medical care subsidy eligibility certificate and National Health Insurance card at the reception desk of the medical institution) Apply within 14 days of the next day after a new birth or moving to Chuo City Application form, the child's National Health Insurance card, personal seal, and bank account number of the child's guardian

Monoparental family medical care subsidy
The parent and child or children in a monoparental family or child without parents is eligible to receive free medical care at medical institutions located in Yamanashi Prefecture. Please note that only medical care costs covered under National Health Insurance are eligible.
Please contact the Child-rearing Support Division prior to applying for your medical care eligibility certificate.
Eligible Persons Scope of Subsidy Subsidy Conditions Required Documents
City residents who meet either of the following conditions:
  • A monoparental household consisting of a mother or father and child or children
  • A guardian that has no spouse and the child or children without parents that they are caring for
  • A child without parents
Medical care costs covered under National Health Insurance (Please show your monoparental family medical care subsidy eligibility certificate and National Health Insurance card at the reception desk of the medical institution.) City residents who meet all of the following conditions:
  • Both the parent/guardian and child or children maintain an address in Chuo City and are enrolled in National Health Insurance
  • The applicant (father, mother or guardian) must be an income tax exempt household
  • Legal dependents (directly related brothers or sisters within the 3rd degree of relationship) residing at the same address fall within the income restriction requirements for the household
Copy of the family register (that includes date of divorce, etc.), copy of the residence registry, personal seal, copies of the National Health Insurance card of all family members, income tax certificate (for those without a residence registry for Chuo City as of January 1)
*Here “child” refers to the period up until the final day of the fiscal year in which a city resident turns 18.
*If you forget your medical care eligibility certificate or were attended by a medical institution located outside Yamanashi Prefecture, please file an application for a reimbursement within two months of the month of the visit.
*City residents receiving the subsidy must complete renewal procedures each August.

Infertility treatment cost subsidy
The city provides a subsidy to partially offset the cost of specific fertility treatments (in vitro fertilization and micro fertilization not covered by National Health Insurance) in order to reduce the economic burden on married couples seeking to have children.
Eligible Persons Subsidy Amount Application Period Required Documents
City residents who meet all of the following conditions:
  • Married couples that have resided at an address in Chuo City for at least 1 year at the time of the application
  • Legally married couples that have been diagnosed by a doctor as being unable to or having very little possibility of becoming pregnant through treatment methods other than specific infertility treatments (in vitro fertilization and micro fertilization)
Up to a maximum of 100,000 yen per year for a total of five years. Submit the required application form and other required documents to Chuo City within 1 year of completing the treatment. Application form, infertility treatment cost subsidy exam certificate (completed by the attending doctor, with attached copy of receipt for specific infertility treatment costs), residence registry or extract (with relationship indicated) (or alien registration card)

Yamanashi Prefecture elderly medical care subsidy
Yamanashi Prefecture offers a subsidy to eligible elderly residents to partially offset the self-pay amount of medical expenses paid at medical institutions in Yamanashi Prefecture.
Eligible Persons Subsidy Amount
City residents who meet all of the following conditions:
  • Persons between the ages of 68 and 69 with an address in Chuo City
  • Persons who are enrolled in National Health Insurance
  • Households where all members are tax exempt residents
*Excluding those insured under the medical care system for late-staged elderly.
The self-pay amount will be 10% for medical care costs covered by National Health Insurance incurred at medical institutions located in Yamanashi Prefecture. (Self-pay ratio as of April 1, 2011.) (Please show your Yamanashi Prefecture elderly medical care subsidy eligibility certificate and National Health Insurance card at the reception desk of the medical institution.)

健康相談・健康教育
保健師による健康相談「すこやか相談」を、玉穂および田富の健康管理センターにおいて行っています。
また、生活習慣病の予防や健康増進に関する知識と技術の普及を図るため、地域に出向いて健康教育を実施しています。
また、ご希望により、健診結果などを書き込める健康手帳を交付しますので、健康管理にご活用ください。

医療費の助成を受けたいとき

出産育児一時金
国民健康保険被保険者が出産をしたときに支給されます。
対象者 支給額 申請手続き(直接支払い制度)
健康保険の被保険者または被扶養者で、妊娠4か月(85日)以上で出産した方(死産、流産等を含む) 42万円(平成23年4月1日現在)
(産科医療保障制度に加入する医療機関などにおける在胎週数22週に達した日以後の出産以外の場合は、39万円。)
医療機関などに保険証などを提示し、直接支払い制度の合意文書を取り交わしてください。
※分娩費用が支給額を下回った場合は、各保険者への申請により、差額が受けられます。
※直接支払い制度を利用しない場合は、各保険者への申請により、出産後、被保険者に支給されます。

重度心身障害者医療助成制度
重度心身障害者が病院にかかったとき、健康保険診療の自己負担分が助成されます。この助成金の支給を受けようとする対象者又は、その保護者は受給者証の交付を申請してください。

子ども医療費助成
小学校6年生までのお子さまの、県内医療機関での医療費(保険診療分)が、窓口で無料になります。
対象者 助成内容 申請期間 必要なもの
0歳〜小学校6年生(小学校6年生の年度末)までのお子さま 保険診療分の医療費(「子ども医療費助成金受給資格者証」と「健康保険証」を窓口で提示してください。) お子さまの生まれた日や転入した日の翌日から14日以内 申請書、お子さまの健康保険証、印鑑、保護者の銀行などの口座番号

ひとり親家庭医療費助成
ひとり親家庭の親と児童や父母のいない児童の、県内医療機関での医療費(保険診療分)が、窓口で無料になります。
受給資格者証の交付申請をする前に、子育て支援課にご相談ください。

以下のいずれかを満たす方
  • ひとり親家庭の父または母と児童
  • 配偶者のいない養育者と養育する父母のいない児童
  • 父母のいない児童
保険診療分の医療費(「ひとり親家庭医療費助成金受給資格者証」と「健康保険証」を窓口で提示してください。) 以下のすべてを満たす方
  • 保護者、児童ともに中央市に住所があり、医療保険に加入している
  • 申請者(父または母、養育者)が、所得税非課税世帯である
  • 同居する扶養義務者(3親等内の直系血族兄弟姉妹)が、所得制限内である
戸籍謄本(離婚日等の記載があるもの)、住民票謄本、印鑑、家族全員の保険証の写し、所得課税証明書(中央市に1月1日に住民票のない方)
※児童とは、18歳の誕生日を迎えた年度の末日までの方です。
※受給資格者証を忘れた場合や県外医療機関で受診した場合などは、償還払い申請をしてください。(診療月から2年以内)
※助成を受けている方は、毎年8月に、必ず更新手続きをしてください。

不妊治療費助成
特定不妊治療(保険外診療の体外受精および顕微授精)を受けられたご夫婦の経済的負担を軽くするため、費用の一部を助成しています。
対象者 助成内容 申請期間 必要なもの
以下のすべてを満たす方
  • 申請時に1年以上中央市に住所を有するご夫婦
  • 法律上婚姻をしており、特定不妊治療(体外受精および顕微授精)以外の治療法によっては妊娠の見込みがないか極めて少ないと医師に判断されたご夫婦
10万円(年額)を限度に、通算5年間支給されます。 治療終了日から1年以内に、申請書等を市に提出する。 申請書、不妊治療費助成金受診等証明書(主治医が記入。特定不妊治療費の領収書の写しも添付)、住民 票もしくは抄本(続柄記載)(または外国人登録証明書)

山梨県老人医療費助成制度
医療機関等に支払う自己負担分の一部を県が助成します。
対象者 助成額
以下のすべてを満たす方
  • 中央市に住所を有する、68歳・69歳の方
  • 健康保険に加入している方
  • 同じ世帯の全員が住民税非課税の方
※後期高齢者医療制度の被保険者は除く。
県内医療機関での医療費(保険診療分)の窓口負担が、1割となります。(負担割合は、平成23年4月1日現在)(「山梨県老人医療費受給者証」と「健康保険証」を窓口で提示してください。)
