Programs for the Elderly and Persons with Disabilities

Comprehensive consultations and support for the elderly (Regional Comprehensive Support Center)
The Regional Comprehensive Support Center provides comprehensive support services related to medical and nursing care to the elderly and their families living in the community. The Center is open to all in need of assistance. Please contact the Center for more information.
  • Provides comprehensive consultations and assistance to the elderly
  • Helps prevent the need for nursing care services (creates plans and holds classes on how to prevent the need for nursing care services, provides health education, supports family members providing nursing care services, offers the Fureai Salon [formerly, Ikigai Day Services], etc.)
  • Provides consultations on preventing elderly abuse and advocates for elderly rights (offers support services to protect the rights of the elderly, including prevention and quick detection of abuse, and accommodates the adult guardianship system, etc.)
  • Provides support to care managers and helps build local community networks

Emergency medical information kit
City residents can fill in information needed during an emergency (illnesses, medications, family doctor, emergency contact information, etc.) using the emergency medical information kit. By storing this kit in the refrigerator, emergency medical technicians have easy access to vital information in an emergency.

Consultation window for city residents with disabilities
A consultant from the Yamanashi Prefectural Government is available to provide various consulting services to city residents with disabilities. The consultation window is open to all in need.

Disability ID card
The disability ID card enables city residents with disabilities to receive various support services.
Physical disability ID card (Level 1 - Level 6): Issued to city residents with some form of permanent physical disability that is classified in the physical disability grade table.
Medical treatment ID card (A or B): Issued to city residents that are determined to have an intellectual disability by a child counseling center or a consultation office for rehabilitation of the disabled.
Mental disability health and welfare ID card (Level 1 - Level 3): Issued to city residents with a mental illness (excluding those with an intellectual disability) and that have been restricted from social activities and daily life for an extended period of time.

In-home welfare services
In-home welfare services are provided to assist city residents with disabilities in their home. These services include short stay and day services provided by facilities as well as the dispatch of home helpers, bathing services and home-delivered meals, etc.

Nursing care insurance
City residents that require nursing care or other forms of assistance because they have become bedridden or are suffering from dementia are eligible to receive nursing care services, including in-home care and in-facility care. Nursing Care Insurance requires a 10% self-pay to receive these services.
Category Eligibility for Enrollment Eligibility to Receive Services Insurance Premiums and Payment Methods
No. 1 Insured Persons over the age of 65 with an address in Japan Persons certified as requiring nursing care and/or assistance Insurance premiums are calculated using a 6-level system based on income.
When the old-age (retirement) pension is more than 180,000 yen per year (15,000 yen per month): Special tax collection (deducted from the old-age pension) Cases other than special tax collection: Ordinary tax collection (payment slip or direct debit)
No. 2 Insured Persons between the ages of 40 and 64 with an address in Japan and enrolled in National Health Insurance Persons certified as requiring nursing care and/or assistance due to an illness (the 15 types of specific illness) related to aging such as presenile dementia or cerebral stroke, etc. Insurance premiums are calculated based on the method determined by the insurance provider.
Lump-sum payment. (The insurance provider will collect the payment as an insurance premium.)
Step-by-step procedures for accessing nursing care services
1. Apply to the city to become certified as requiring nursing care → 2. Receive screening as to the extent of nursing care services that are required → 3. Have an in-home nursing care provider create a care plan up to the maximum amount allowed as determined by the screening → 4. Receive nursing care services according to the care plan → 5. Pay 10% of the costs as the self-pay amount
Nursing care service types
In-home services: Includes in-home care provided by a home helper such as nursing care and help with housekeeping, and home remodelling for nursing care purposes where differences in floor level are renovated and handrails are installed, etc.
In-facility services: Includes nursing care services and assistance with daily needs at a welfare facility for the elderly and medical nursing or rehabilitation at a healthcare facility for the elderly, etc.

救急医療情報キット
救急時に必要な情報(持病や常服薬、かかりつけ医、緊急連絡先など)を記入し、冷蔵庫の中に保管しておくことで、万一の緊急時に、救急隊員が適切かつ迅速な対応をとることができます。

障害者手帳
障がいのある方は、障害者手帳により、各種支援やサービスを受けることができます。
身体障害者手帳(1級〜6級):身体に永続的な一定の障がいがあり、障がいの程度が身体障害者程度等級表に該当する方
療養手帳(A・B):児童相談所または障害者相談所において知的障がいと判定された方
精神障害者保健福祉手帳(1級〜3級):精神疾患を有する方(知的障がい者を除く)で、長期にわたり日常生活または社会活動に制約がある方

在宅福祉サービス
障がいのある方を居宅において援助するための福祉サービスで、施設機能を利用したショートステイやデイサービスの他、ホームヘルパーの派遣、入浴サービス、配食サービス等があります。

介護保険
寝たきりや認知症などで介護や支援が必要な状態になった場合、訪問介護や通所介護などの介護サービスを、費用の1割(原則)負担で受けることができます。
区分 加入対象者 受給対象者 保険料と納付方法
第1号被保険者 市町村内に住所をもつ65歳以上の方 要介護・要支援認定を受けた方 保険料額は、所得に応じて6段階あり。
老齢(退職)年金が年額18万円(月額1万5,000円)以上の場合:特別徴収(年金からの天引き)特別徴収以外の場合:普通徴収(納付書や口座振替)
第2号被保険者 市町村内に住所をもつ40歳以上65歳未満の医療保険加入者 初老期の認知症や脳卒中等、老化にともなう病気(15種類の特定疾病)により要介護・要支援認定を受けた方 保険料額は、加入医療保険の算定方法による。
一括納付。(医療保険者が医療保険料として徴収)
介護サービス利用の手続き
市に「要介護認定」を申請 → どの程度の介護が必要な状態か認定を受ける → 定められた限度額の範囲内で居宅介護支援事業者に「ケアプラン」を作成してもらう →「ケアプラン」に沿って、介護サービスを利用する → 費用の1割(原則)を支払う
介護サービスの種類
居宅サービス:ホームヘルパーによる介護や家事援助などの「訪問介護」、手すりの取り付けや段差の改修などの「居宅介護住宅改修」など
施設サービス:「介護老人福祉施設」での介護や日常生活上の世話、「介護老人保健施設」での医学的な看護やリハビリなど
