National Health Insurance

Persons that are not enrolled in any health insurance are by law required to enroll in the National Health Insurance. Enrollment procedures must be completed within 14 days of becoming eligible.

*Personal identification document refers to an official photo document such as a driver's license, basic resident's registration card, or a passport.

High-cost medical treatment expenses National Health Insurance refunds a portion of the self-pay amount for high-cost medical treatment expenses. (excluding the cost for hospitalization in a single patient room and meals, etc. There are certain exceptions for enrollees under the age of 70.) Medical treatment expenses If you did not have your National Health Insurance card when receiving treatment or received medical treatment at a clinic that does not offer healthcare services provided by health insurance, such as a chiropractor, you may be entitled to receive a refund in an amount excluding the self-pay amount, if your application is accepted. Application form, statement of medical services received (health insurance claim), receipts, National Health Insurance card, personal seal, etc.

Funeral expenses 50,000 yen will be paid to the person who arranges the funeral of a National Health Insurance enrollee who passes away. (As of April 1, 2011) Personal seal and bank account number for the remittance of the funeral expenses

National Health Insurance premiums
National Health Insurance premiums are calculated based on the total of the medical care insurance portion, late-stage elderly medical care assistance portion and nursing care contribution portion for National Health Insurance enrollees up to the age of 74.
*Please contact the office listed below to learn more about programs that reduce the cost of National Health Insurance premiums for city residents with low income.
Insurance premium payment methods
National Health Insurance premiums can be paid by direct debit from your bank account or in person at financial institutions, convenience stores, the Tatomi Office / Accounts Division or the Tamaho Office / Desk Service Division.
(Annual premiums are paid in 8 installments between July and February of the following year.)

国民健康保険について

いずれの健康保険にも加入していない方は、国民健康保険への加入が義務付けられています。事実発生後14日以内に手続きをしてください。

生活保護を受けなくなったとき 保護廃止決定通知書、印鑑
子どもが生まれたとき 出産した人の保険証、母子健康手帳、印鑑
脱退 市外へ引越すとき 国民健康保険証、印鑑
職場の健康保険に入ったとき 国保と職場の健康保険の両方の保険証、印鑑
生活保護を受けたとき 国民健康保険証、保護決定通知書、印鑑
死亡したとき 国民健康保険証、印鑑
その他 住所、世帯主、氏名等が変わったとき 国民健康保険証、印鑑
世帯の合併・分離があったとき
子どもが、修学のため他の市町村に住むとき 国民健康保険証、在学証明書、印鑑
保険証をなくしたまたは汚れて使えなくなったとき 本人確認書類、印鑑
納税証明書の交付を受けるとき 本人確認書類(本人以外の場合は委任状)、印鑑
※本人確認書類は、運転免許証や住基カード、パスポートなど、官公署が発行した顔写真付きの書類です。
主な給付

高額療養費 医療費の自己負担額が高額になった場合、支払った金額の一部が支給されます。(差額ベッド代、食事等は対象外。70歳未満の方には支払特例が有ります) 市役所から送付したハガキ、申請書、被保険者証、印鑑、治療費の領収書、銀行口座のわかるもの
療養費 保険証を持たずに治療を受けたときや保険診療の扱いがない接骨院で治療を受けたとき等、申請が認められれば、自己負担分を除いた額が払い戻されます。 申請書、診療内容の明細書(レセプト)、領収書、保険証、印鑑等

葬祭費 被保険者が亡くなったとき、葬祭を行った方に50,000円が支給されます。(平成2年4月1日現在) 印鑑、葬祭費の振込み口座番号

国民健康保険税
74歳までの国民健康保険加入者の医療保険分、後期高齢者支援金分、介護納付金分を合算して算定。
※所得の低い方への軽減措置などについては、お問い合わせください。
保険税の納付方法
金融機関などや、コンビニ、田富庁舎 / 会計課、玉穂庁舎 / 窓口課で、口座振替または直接支払で納税できます。(年税額は7月から翌年2月までの8回に分割)
