Late-stage Elderly Medical Care Insurance

Late-stage Elderly Medical Care Insurance ensures that all residents of Japan will be able to receive medical care during old age.

Eligible persons
All persons over the age of 75 (starting from their 75th birthday)
Persons between the ages of 65 and 74 that have a certain degree of disability, such as being bedridden, and that are certified by the regional late-stage elderly medical care association through an application process (starting from the date certification is granted)
Enrollment, withdrawal and other procedures
Procedures Required When... Required Documents
Enrollment You reach the age of 75 No notification required (an insurance card will be sent by postal mail immediately prior to enrollee's 75th birthday)
You are between the ages of 65 and 74 and have a certain degree of disability Either the enrollee's disability card, pension certificate or doctor's certificate, current National Health Insurance card, personal seal, personal identification documents
You move to Chuo City from outside Yamanashi Prefecture Late-stage elderly medical care classification certificate, personal seal, personal identification documents
You are no longer eligible to receive public assistance Termination of public assistance notice, personal seal and personal identification documents
Withdrawal You are certified as having a disability and the disability no longer is applicable to certification or you apply to withdrawal your certification of disability National Health Insurance card and personal seal
You move outside Yamanashi Prefecture National Health Insurance card and personal seal (a late-stage elderly medical care classification certificate will be issued)
You receive public assistance National Health Insurance card, public assistance commencement notice and personal seal
You die National Health Insurance card and personal seal (the funeral organizer can apply for payment of the funeral expense)
Other You move within Yamanashi Prefecture Within Chuo City: National Health Insurance card, personal seal, personal identification documents A different municipality: Personal seal and identification documents (return the National Health Insurance card to the municipality of your new address)
Your name changes National Health Insurance card, personal seal, personal identification documents
You lose your Late-stage Elderly Medical Care Insurance card or it has been damaged and can no longer be used National Health Insurance card (except when lost), personal seal, personal identification documents
*Personal identification document refers to an official photo document such as a driver's license, basic resident's registration card, or a passport.
Primary benefits
Late-stage Elderly Medical Care Insurance entitles enrollees to medical treatment for injuries or illnesses. Simply present your Late-stage Elderly Care Insurance card at the reception counter of the medical institution. The self-pay amount for enrollees is 10% of total medical treatment costs (30% for individuals with an income above a certain threshold). In addition, a variety of other benefits, such as in-home nursing services and death benefits, can also be received by an enrollee with an application.
Insurance premiums
Insurance premiums for fiscal 2010 and 2011 (revised every two years)
= Fixed Contribution (payable by all enrolees) [38,710 yen] + Income Based Contribution (based on income) [(Income - 330,000 yen) x 7.28%]
*Enrollees with incomes below a certain threshold are eligible for reduced insurance premiums by application. (Reduction or exemption of the fixed contribution and income based contribution due to accident or decline in income, etc.)
Insurance premium payment methods
Persons with pension payments exceeding 180,000 yen per year: Special tax collection (deducted from pension)
Cases other than special tax collection: Ordinary tax collection (financial institutions, Japan Post Bank, convenience stores [payment via payment slip] or direct debit from your bank account)

後期高齢者医療保険について

将来にわたり安心して医療を受けられるよう、国民みんなで支える仕組みです。

対象者
75歳以上のすべての方(75歳の誕生日当日から)
65歳以上75歳未満の方で、寝たきりなどの一定の障害があり、申請により保険者(後期高齢者医療広域連合)の認定を受けた方(認定日から)
加入・脱退・その他の手続き
種類 必要なもの
加入 75歳になったとき 届出不要(誕生日直前に保険証が郵送されます)
65歳以上75歳未満の方で、一定の障害があるとき 身障者手帳・年金証書・診断書のいずれかの書類、現在加入している保険証、印鑑、本人確認書類
県外から引越ししてきたとき 後期高齢者医療負担区分証明書、印鑑、本人確認書類
生活保護を受けなくなったとき 保護廃止(休止)決定通知書、印鑑、本人確認書類
脱退 障害認定を受けている方で、障害状態不該当になったときまたは障害認定の撤回の申請をするとき 被保険者証、印鑑
県外へ引越すとき 被保険者証、印鑑
(後期高齢者医療負担区分証明書をお渡しします)
生活保護を受けたとき 被保険者証、保護開始決定通知書、印鑑
死亡したとき 被保険者証、印鑑
(葬儀執行者は、葬祭費の申請ができます)
その他 県内で引越ししたとき 市内の場合:被保険者証、印鑑、本人確認書類
他市町村の場合:印鑑、身分証明書(被保険者証は転出元の市町村に返還)
氏名が変わったとき 被保険者証、印鑑、本人確認書類
被保険者証をなくしたまたは破損等したとき 被保険者証(紛失以外の場合)、印鑑、本人確認書類
※本人確認書類は、運転免許証や住基カード、パスポートなど、官公署が発行した顔写真付きの書類です。
主な給付
病気やケガの場合、被保険者証を医療機関などの窓口に提出すると、自己負担額は費用の1割(一定以上の所得がある方は3割)となります。その他、被保険者が亡くなった場合や訪問看護サービスを受けた場合などに、申請によりさまざまな給付を受けることができます。
保険料
平成22・23年度の保険料(2年ごとに見直し)
= 均等割額(被保険者が等しく負担)【38,710円】+ 所得割額(所得に応じる)【(所得 - 33万円)× 7.28%】
※所得が一定額以下の場合など、保険料の軽減などが受けられる場合があります。(均等割額・所得割額の軽減、災害や収入の減少による減免など)
保険料の納付方法
年金が年額18万円以上の場合:特別徴収 年金からの天引き
特別徴収以外の場合:普通徴収金融機関・ゆうちょ銀行・コンビニ(納付書で納付)、口座振替で納付してください。
